What is Care Coordination?
It's a program designed to provide support and case management services to our patients.
What does a Care Coordinator Do?
The Care Coordinator addresses other appropriate needs as identified by ANY hospital staff member. This can include but is not limited to:
- No health insurance, underinsured, special enrollment circumstance such as marriage, birth of child
- Lack of prescription drug coverage
- Difficulty attending appointments, lack of transportation
- No medical home
- Chronic medical condition
- Need for behavioural health or substance abuse counseling and treatment
- Narcotic seeking behaviours
- Failure to comply with discharge instructions, preventactive care measures
