It's a program designed to provide support and case management services to our patients.

The Care Coordinator addresses other appropriate needs as identified by ANY hospital staff member. This can include but is not limited to:

No health insurance, underinsured, special enrollment circumstance such as marriage, birth of child
Lack of prescription drug coverage
Difficulty attending appointments, lack of transportation
No medical home
Chronic medical condition
Need for behavioural health or substance abuse counseling and treatment
Narcotic seeking behaviours
Failure to comply with discharge instructions, preventactive care measures
